About the Role
The Older Youth Case Manager works with a specialized caseload of adolescent youth to promote successful transitions into independent adulthood. This role focuses on coordinating independent living services, conducting assessments, attending court, maintaining documentation, and ensuring eligible youth receive services at key transition points outlined in policy.
This position is primarily based in Tell City and serves Perry and Dubois Counties. Mileage is reimbursed, and augmented pay is provided for on-call hours.
Key Responsibilities

Investigate reports of child abuse, neglect, or dependency and recommend appropriate actions

Assess risk to youth and determine safety needs, including removal when necessary

Conduct needs assessments and recommend appropriate treatment or support services
-
Prepare and present court reports, including pre-dispositional and progress reports
-
Develop and monitor case plans focused on independence, safety, and long-term stability
-
Coordinate independent living and transition services for older youth
-
Develop discharge and transition programming to support self-sufficiency
-
Monitor service providers to ensure alignment with case plans
-
Accept on-call responsibilities related to child abuse and neglect allegations

Conduct home studies and family histories as needed

Visit youth in out-of-home placements to support reunification, adoption, or independent living

Maintain accurate case documentation and data for program evaluation
Qualifications
Applicants must meet one of the following requirements:
-
Bachelor’s degree, or
-
Associate degree plus two (2) years of human services experience, or
-
Thirty (30) college credit hours plus four (4) years of human services experience, or
-
A minimum of four (4) years of human services experience
Additional Knowledge & Skills

Knowledge of social work, case management, counseling, and behavioral sciences

Knowledge of state and federal child welfare laws and regulations
-
Ability to investigate sensitive situations and conduct effective interviews

Strong judgment and crisis management skills
-
Ability to prepare clear, accurate reports and documentation
-
Ability to work effectively with youth and families from diverse backgrounds
-
Ability to work with youth with special needs and coordinate with service providers
-
Valid driver’s license required

Salary

Starting salary: $47,320, with potential adjustments based on education and experience
