Perry County Memorial Hospital is proud to offer quality care beyond our hospital walls. We’re currently hiring a Full-Time Home Care Physical Therapist based out of our Tell City Clinic. This is a rewarding opportunity to help patients regain mobility and independence in the comfort of their own homes.

About the Role:
As a Home Care Physical Therapist, you will evaluate and treat patients with physical impairments, functional limitations, or disabilities due to injury or illness. You’ll play an essential role in developing treatment plans that support safe, effective rehabilitation and help patients achieve their goals. This position requires strong decision-making, stamina, and a commitment to delivering care with compassion and professionalism.

Key Responsibilities:

• Assess patient physical function and develop individualized care plans

• Deliver physical therapy treatments to improve mobility, strength, and coordination

• Monitor and adjust therapy plans based on patient progress and response

• Educate patients and families on exercises and care plans to support recovery

• Maintain accurate documentation in accordance with clinical and regulatory standards

• Communicate effectively with physicians, nurses, and other care team members

Qualifications:

• Graduate of an accredited school of physical therapy

• Licensed to practice as a Physical Therapist in the State of Indiana

• Current certification in CPR

• Ability to make independent clinical decisions and adapt to various home environments

• Physical stamina and strength to lift, transfer, and support patients as needed

• Strong interpersonal skills and the ability to work independently in the field

Work Environment:
This is an on-site position based at the Tell City Clinic, with regular travel to patient homes throughout the area. The role involves physical exertion and working in varying home conditions, requiring excellent use of body mechanics and clinical judgment.
