About the Role:
As the Assisted Living Coordinator, you’ll direct and manage the health and personal care needs of each Assisted Living resident. You’ll oversee staff, coordinate care plans, monitor resident wellness, and serve as the critical link between residents, families, healthcare providers, and the campus team—all while ensuring a safe, supportive, and engaging environment.

Key Responsibilities:

• Complete pre-admission assessments and monitor ongoing resident health needs

• Develop and update personalized care and service plans with residents and families

• Supervise and train Nursing Assistants (CNA, QMA) and licensed clinical staff

• Manage pharmacy and medication processes to ensure regulatory compliance

• Facilitate communication among residents, families, healthcare providers, and the campus

• Lead Assisted Living resident meetings and promptly address concerns

• Maintain accurate medical records and documentation per state regulations

• Participate in campus committees, marketing initiatives, and referral outreach

• Provide weekend coverage and on-call management on a rotating basis

• Assist in recruitment, onboarding, and continuous staff training efforts

What You’ll Need:

• Licensed Practical Nurse (LPN) degree from an accredited nursing school

• Current, unencumbered LPN license in Indiana

• Valid CPR certification

• Valid driver’s license and ability to operate a large van or 15-passenger vehicle

• Experience working in Assisted Living, Long-Term Care, or Senior Services preferred

• Strong leadership, communication, and organizational skills
