Home Care Staff RN

About the Role

The Home Care Staff Registered Nurse delivers professional nursing services in patient homes, requiring strong clinical judgment, independence, and adaptability. This role supports patients across varying conditions while coordinating care, educating patients and families, and ensuring safe, effective treatment in a home-based setting.

Key Responsibilities

• Provide skilled nursing care to patients in their homes

• Perform patient assessments and develop appropriate care plans

• Make independent clinical and care management decisions related to patient care

• Educate patients and families on care plans, medications, and health management

• Maintain accurate and timely documentation

• Use proper body mechanics when lifting or transferring patients

• Respond effectively to adverse or changing patient conditions

• Participate in on-call rotation as required

• Ensure compliance with all clinical, safety, and regulatory standards

Qualifications

Education

• Graduate of an accredited school of nursing required

Licensure & Certification

• Current Registered Nurse (RN) license in the State of Indiana required

• Current BLS/CPR certification required

• Must maintain valid driver’s license and required renewals

• Must provide proof of current automobile insurance for liability, personal injury, death, or property damage

• Must provide own reliable transportation to and from patient homes

Experience

• Minimum of two (2) years of recent clinical experience preferred

• Public health, home health, or hospice experience strongly preferred

Physical & Mental Requirements

• Requires close attention to detail and independent decision-making

• Requires alertness, adaptability, and sound clinical judgment

• Work performed in sitting and standing positions

• Occasional physical exertion and stamina required

• Ability to work effectively under adverse or changing conditions

• Proper body mechanics required for lifting and transferring patients
