About the Role
As a Home Care / Inpatient Occupational Therapist at Perry County Memorial Hospital, you will play an essential role in delivering hands-on, individualized care in both clinical and home settings. You’ll help patients achieve their highest functional potential through rehabilitative strategies tailored to each individual’s needs. This position requires both clinical expertise and the flexibility to work independently in patients’ homes. You will also collaborate with healthcare teams and families to support patient progress, safety, and comfort.
Key Responsibilities
- Assess, plan, and implement individualized occupational therapy programs for patients in both inpatient and home care settings.
- Guide patients in therapeutic exercises and activities to improve strength, coordination, and daily living skills.
- Maintain accurate and timely clinical documentation and reports.
- Communicate and coordinate care with physicians, nurses, and other therapy team members.
- Educate patients and caregivers on continued therapy and home safety modifications.
- Travel to patient homes as scheduled, adhering to safety and documentation protocols.
Qualifications
We’re looking for candidates who meet the following criteria:
- Graduate of an approved Occupational Therapy program.
- Indiana state licensure as an Occupational Therapist.
- Current certification in BLS/CPR.
- Valid driver’s license, reliable transportation, and proof of auto insurance (for home care).
- Previous experience in occupational therapy preferred.
- Ability to work independently with sound judgment and decision-making skills.
